All vaccination providers are from January 14, 2021 till further notice ordered to record the logistics of vaccines pursuant to this Measure set forth as follows:
I.
All those medical service providers, in whose premises there is, pursuant to the Vaccination Campaign Methodical Advice {Realisation Plan), a Vaccine Distribution Hub, a Large-Capacity Vaccination Point or a Vaccination Point set up (henceforth as "Provider"), are with effect from January 14, 2021, 12:00am, ordered to continuously provide information on the amount of vials
a) the Provider has received,
b)  the Provider has taken in stock in order to hand them over to another provider,
c) the Provider has taken in stock in order to use them up in his own facility
upon each acceptance or dispensing of the vaccine.
This information be provided to the National Vaccination Coordination Point using the COVID Forms App reporting tool (http://cfa.uzis.cz/ - in Czech only) and its "Material Logistics" module (Czech: Logistika materiálu).

II.
All Providers are with effect from January 14, 2021, 12:00am ordered to continuously provide information on the amount of
a) vials,
b) needles,
c) syringes,
d) saline (10ml or more)

in stock upon any change of these amounts. This information be provided to the National Vaccination Coordination Point using the COVID Forms App reporting tool (http://cfa.uzis.cz/ - in Czech only) and its "Material Logistics" module (Czech: Logistika materiálu).

III.

All Providers are with effect from January 14, 2021, 12:00am ordered to continuously provide information in scope as defined in the Covid Forms App (http://cfa.uzis.cz/ - in Czech only) - module "Vaccination Point" (Czech: Očkovací místo), item "Vaccination Point Entry" (Czech: Záznam - očkovací místo)

IV.

All Providers are with effect from January 14, 2021, 12:00am ordered to immediately update the data defined in the Covid Forms App (http://cfa.uzis.cz/ - in Czech only) - module "Vaccination Point" (Czech: Očkovací místo), item "Vaccination Points" (Czech: Očkovací místa). To fulfill this order, General Practitioners among Providers are obligated to carry out a registration process within the pool of their registered patients who had expressed interest in getting vaccinated, this registration process has to be carried out using the Infectious Diseases Information System (Czech: Informační systém infekčních nemocí, ISIN).

V.

All Providers, except of General Practitioners, are with effect from January 14, 2021, 12:00am ordered to use the Central Reservation System as the electronic countrywide COVID-19 vaccination management tool to book timeslots for individuals having expressed interest in getting vaccinated.

VI.

All Providers are with effect from January 14, 2021, 12:00am ordered to act pursuant the Vaccination Plan issued by the Ministry of Health of the Czech Republic when carrying out the vaccination against COVID-19.
