When arriving from risky places, fill in the arrival form and undergo the RT-PCR test for COVID-19 at your own expense.

According to point I (only in Czech), the Ministry of Health orders:

Countries with low risk (green countries) and medium risk of infection (orange countries):

1. all persons who have stayed more than 12 hours in the last 14 days in countries or territories on the list of countries or territories at low or medium risk of COVID-19,

a) complete an Arrival Form before entering the Czech Republic,

(b) undergo an RT-PCR or RAT test before entering the Czech Republic or no later than 5 days after entering the Czech Republic,

(c) produce, upon request at border or residence control, proof of completion of the Arrival Form or written proof that the person concerned is covered by an exemption under this protection measure;

see Section III.2 for the definition of the Arrival Form and Section III.3 for the test requirements;

2. that the obligations set out in point I.1 do not apply to the persons listed in point I.5; exceptions to the sub-obligations are set out in points I.6 to I.10;

Arrivals from countries at high (red countries) and very high risk of infection (dark red countries)

3. all persons who have stayed more than 12 hours in the last 14 days in countries or territories on the list of countries or territories at high or very high risk of COVID-19,

a) complete an Arrival Form before entering the Czech Republic,

(b) have an RT-PCR test result before entering the Czech Republic;

(c) provide, on request at border or residence control, proof of completion of the Entry Form, the result of a SARS-CoV-2 test, or written proof that the person concerned is exempted under this protection measure,

(d) undergo an RT-PCR test between the 5th and 7th day after entering the Czech Republic, unless the public health authority has decided otherwise in exceptional circumstances,

(e) at all times when leaving the place of residence or accommodation, wear a respirator or similar device (without an exhalation valve) fulfilling at least all technical conditions and requirements (for the product), including a filtration efficiency of at least 94 % according to the relevant standards (e.g. FFP2, CN 95), until the negative result of the RT-PCR test referred to in point (c);

the definition of the Arrival Form is given in point III.2 and of the test requirements in point III.3;

4. that the obligations set out in point I.3 do not apply to the persons referred to in point I.5; exceptions to the sub-obligations are set out in points I.6 to I.10;

THESE CONDITIONS FOR ENTRY INTO THE CZECH REPUBLIC DO NOT APPLY TO:

According to point I/C (only in Czech), the Ministry of Health stipulates:

C.a) Exceptions to all anti-epidemic measures under this protective measure

5. that the obligations resulting from the established anti-epidemic measures under this protection measure that apply to arrivals from countries with a low, medium, high, and very high risk of COVID-19 do not apply to

(a) international transport workers provided that the reason for entry is supported by appropriate documentation,

(b) persons authorized to enter the Czech Republic who

(i) are in transit for up to 12 hours through the Czech Republic by land,

(ii) travel by land to or from the Czech Republic for a period not exceeding 12 hours; in the case of travel from third countries with a very high risk of infection, travel to the Czech Republic under this point may be made only for the reason of necessary travel referred to in III.10(a) to (f),

(iii) travel by land directly to or from neighboring countries for a period not exceeding 24 hours;

(c) accredited members of diplomatic missions in the Czech Republic, including private service persons, holders of service passports issued by the Czech Republic and holders of diplomatic passports traveling to or from the Czech Republic on official business, and officials of international organizations registered with the Ministry of Foreign Affairs traveling to or from the Czech Republic on official business, provided that their stay on or outside the territory does not exceed 72 hours,

d) minors with intellectual disabilities, autism spectrum disorder, cognitive disorder, or severe mental alteration, whose mental abilities or current mental state do not allow compliance with this protection measure and who have a written medical certificate to this effect issued in English by a doctor operating in the Czech Republic or in another Member State of the European Union; the certificate must contain a stamp, the doctor's identification data and his/her telephone number,

(e) cross-border workers, pupils, and students who regularly cross the state border with the Czech Republic to or from a neighboring state for the purpose of work or education, and for traveling to or from a neighboring state for the purpose of exercising the right of care or access to a minor child or for the purpose of visiting a spouse or registered partner who is employed or studying in the neighboring state,

(f) members of the Prison Service or police officers carrying out escort activities or security escorts for aircraft,

(g) cross-border cooperation in the context of an intervention by the integrated rescue system,

(h) children up to the age of 12 years;

C.b) Exceptions to certain anti-epidemic measures under this protective measure

6. that vaccinated persons with a national certificate of vaccination, vaccinated persons with a national certificate of completed vaccination and persons who have contracted COVID-19 are only obliged to complete an Arrival Form before entering the Czech Republic and to present proof of completion of the Arrival Form upon request at border or residence control; who is considered a person under this point is specified in point III of this protection measure;

7. that the obligation to have the result of the RT-PCR test prior to travel does not apply to holders of a diplomatic note issued by the Ministry of Foreign Affairs stating that it is impossible to secure a test for the presence of SARS-CoV-2 virus outside the territory of the Czech Republic;

8. that in the case of arrival by means of public transport, only the result of the RAT test may be held before entering the territory of the Czech Republic instead of the result of the RT-PCR test, and that in the case of arrival by means other than public transport, the obligation to hold a SARS-CoV-2 test before arrival shall not apply in the case of

(a) citizens of the Czech Republic and their family members traveling with a temporary residence permit issued for the Czech Republic,

(b) citizens of the European Union who have been issued with a certificate of temporary residence in the Czech Republic,

c) foreign nationals with a permanent residence permit issued by the Czech Republic;

9. that in the case of journeys by public transport directly to or from neighboring countries for a period not exceeding 24 hours, only the obligation to have a test result, which may be the result of the RAT test, before the journey applies;

10. that critical infrastructure service personnel whose activities are in the interest of the Czech Republic are not subject to the obligation to undergo a test on the territory of the Czech Republic; however, they are obliged to wear a respirator or similar device (without an exhalation valve) fulfilling at least all technical conditions and requirements (for the product), including a filtration efficiency of at least 94% according to the relevant standards (e.g. FFP2, CN 95), for a period of 14 days after entering the territory of the Czech Republic;
