Vaccination of children and minors (5-11 and 12-17 years old)

Tip: Properly check and remember the data (phone number, email, name order) you fill in about the child on the spot and if they are the same as those filled out at registration. By doing that, you minimalize the probability of problems occurring when downloading the certificate. For example, it can happen that at registration you fill in the email of one parent and use email of the other parent at the sampling point.

Minors need to be accompanied to the sampling point by their legal representative, who provides an informed consent with the vaccination (paper form is not necessary, but the sampling point can ask for it).

The legal representative needs to also fill in the Pre-vaccination questionnaire (5-11 years and 12-15 years). This is used by the physician at the sampling point to assess the health of minor. The legal representative needs to have their ID and the minor’s healthcare insurance card oncthem. It is also recommended to have a document proving that they are indeed the legal representative of the minor e.g., the minor’s Birth certificate.
Persons who are 16 or older do not need to be accompanied by their legal representative and can fill out the questionnaire themselves.

You can download the certificate at

The most practical is to download the Tečka app through which you can then download the certificate. Minors can have the certificate in the Tečka app on their phone. The legal representative can also add the minor into their Tečka app on their phone, where they can already have their certificate.

  • How to use Tečka
  • How to download the certificate

Problems with downloading the certificate for minors are mostly caused when the parent enters their email, and the other parent is then trying to download it through their email etc. Please remember what data you fill in at the vaccination and use those when downloading the certificate.
