Vaccination of minors (5 - 11 years and 12 - 15 years)

Tip: Check and remember the personal data (such as your telephone number, email, or name order) are filled in about the child at the place and if they are in accordance with the data you used at the registration. You minimize the probability of difficulties that can happen at the later downloading of the certificate. It can happen, for example, that you fill in an email of one parent at the registration but use an email of the other parent at the vaccination center.

Minors need to be accompanied by their legal representative who gives their informed consent to vaccination (it is not necessary to have such consent in the written form; however, the vaccine center can request it).

The legal representative also fills in the Pre-Vaccination Questionnaire (5 to 11 and 12 to 15) which helps the vaccine centre physician with the health status assessment of the minor. They also need to bring their ID and minor’s insurance card. To be safe, also bring a document certifying that you are the legal representative of the minor e.g., their birth certificate.

People aged 12 to 15 years old are vaccinated by Comirnaty vaccine (Pfizer/BioNTech), for people 12+ there is also an approved Spikevax vaccine (Moderna) and Nuvaxovid (Novavax).

For children aged 5 to 11 years old, available vaccination centers will appear at the registration in the Central Reservation System. They are vaccinated with a lower dose of Comirnaty vaccine (Pfizer, BioNTech).

More about vaccines

At you can filter the vaccine center for people aged 12 to 15 by a type of vaccine, or the applicant’s age and place of living.

For children aged 5 to 11 years old, available vaccination centers will appear at the registration in the Central Reservation System (the same as for adults), paediatricians will also be vaccinating. However, for the paediatrician, an individual agreement is necessary, you cannot register through the Central Reservation System.

Getting the vaccination certificate

You can download the certificate at The most practical way is to download the Tečka application. That way can the minor have their certificate on their phone. The legal representative can also add the minor’s certificate to their Tečka application, where they can already have their own certificate.

Problems with downloading the certificate in minors are most often caused by the accompanying parent entering their e-mail and then the other parent trying
to download it from theirs etc. Please, remember what details you fill in during the vaccination and then download the certificate with these. If you need any
help, contact Infoline 1221.
